If you’ve never had a chiropractic adjustment before – or you’ve had one somewhere else and weren’t sure what to make of it – the idea of coming in for the first time can feel a little uncertain. That’s completely normal, and it’s worth addressing directly. The Evaluation Comes Before Anything Else

The first thing that surprises most new patients is how much time we spend looking before we do anything. A Gonstead evaluation isn’t a quick once-over. It’s a structured 5-component process, and it has to happen before we adjust anything.

We start with your case history – a real conversation about what’s been going on, how long it’s been happening, what makes it better or worse, and what your daily life looks like. This isn’t paperwork for its own sake. The history tells us things the physical exam can’t, and it shapes how we interpret everything we find afterward.

From there, we move into visualization – looking at how you stand, how your posture distributes weight, whether your shoulders or hips sit level. Then instrumentation, which is where the Nervoscope comes in. The Nervoscope is a small instrument that runs along the spine and detects temperature differences between the two sides of each vertebral level. Those temperature differences indicate where nerve interference is present. It doesn’t hurt at all – most people describe it as a light gliding sensation.

Palpation comes next – hands-on assessment of how each spinal segment moves, where there’s restriction, where there’s tenderness, and what the muscle tone tells us about compensation patterns. And then, when indicated, we take full-spine X-rays.

About the X-Rays

Full-spine X-rays are something a lot of first-time patients have questions about. Not every patient requires them on the first visit, but for many new patients, they’re part of what makes a Gonstead evaluation complete.

The reason is straightforward. Palpation and instrumentation tell us a great deal about what’s happening functionally, but X-rays show us the actual structural picture – disc spacing, vertebral positioning, the angles and relationships between segments. That structural information changes what we do. An adjustment that would be appropriate for one structural pattern might be contraindicated for another. We need to see the whole spine in context before we correct it.

Full-spine films also reveal compensation patterns that aren’t obvious from the surface. A problem that shows up as lower back pain might have its root cause two or three levels above where the pain is. Without imaging, that gets missed.

So What Does an Adjustment Actually Feel Like?

Once the evaluation is complete and we know exactly which segments need correction, the adjustment itself is usually faster than people expect. We’re not adjusting the whole spine. We’re correcting the specific segments that the evaluation identified – nothing more.

The position varies depending on which segment is being adjusted. Some are done seated, some lying face-down on the table, some on the side. Dr. Blake Wayson will explain the position before you’re in it and tell you what’s about to happen.

The adjustment itself involves a controlled, specific thrust to the segment. For most patients, there’s an audible pop or click – sometimes one, sometimes a few. That sound is just gas releasing from the joint fluid as the joint surfaces separate slightly. It’s not bones cracking, and it’s not an indication of how well the adjustment worked. Some good adjustments are quiet. Some that sound impressive don’t do much. The sound is just a byproduct of the mechanics.

As for how it feels – most people describe it as a release. There’s a moment of mild pressure, then a sense of movement in the segment, then usually some immediate relief in the surrounding tissue. Some people feel a little vulnerable or shaky for a moment right afterward, which is a normal nervous system response to the change in input. That settles quickly.

The Sounds People Are Actually Nervous About

The popping sound is the thing most first-timers are anxious about before they come in. It almost always sounds worse in people’s imaginations than it does in the room. Once you’ve experienced it, the concern usually disappears entirely.

What actually catches people off guard is how specific it is. If you’ve ever had someone pop your back by giving you a bear hug or twisting you, that general manipulation hits multiple segments at once and sounds like a lot. A Gonstead adjustment is targeted to one segment. The sound is usually a single, clean pop rather than a cascade of cracks.

What Happens in Your Body After the Adjustment

In the hours following your first adjustment, a few different things might happen. Many people notice immediate improvement – less tension, better range of motion, a sense of lightness or ease in the adjusted area. That’s the nervous system responding to the restored alignment.

Some people feel mild soreness in the adjusted area, similar to how muscles feel after exercise. It typically resolves within 24-48 hours and becomes less common as care progresses.
