For Chiro Booster, LLC: These terms relating to Chiro Booster, LLC and website services shall be governed by and construed in accordance with the laws of the State of Ohio, without regard to conflict of law principles. Any disputes relating to Chiro Booster, LLC shall be resolved in the courts of Delaware County, Ohio.

Severability

If any provision of these terms is found to be invalid or unenforceable by a court of competent jurisdiction, the remaining provisions shall remain in full force and effect. The invalid or unenforceable provision shall be replaced with a valid provision that most closely matches the intent of the original provision.

  • You can ask to see or get an electronic or paper copy of your medical record and other  health information we have about you. Ask us how to do this.  
  • We will provide a copy or a summary of your health information, usually within 30 days of  your request. We may charge a reasonable, cost-based fee.  
  • If you request an electronic copy, we will provide the information in the electronic form and  format you requested, if it is readily producible. If it is not readily producible, we will work  with you to agree on a readable electronic format.

 

Ask us to correct your medical record  

  • You can ask us to correct health information about you that you think is incorrect or  incomplete. Ask us how to do this.  
  • We may say “no” to your request, but we’ll tell you why in writing within 60 days.  Request confidential communications  
  • You can ask us to contact you in a specific way (for example, home or office phone) or to  send mail to a different address.  
  • We will say “yes” to all reasonable requests.

Get a list of those with whom we’ve shared information 

  • You can ask for a list (accounting) of the times we’ve shared your health information for six  years prior to the date you ask, who we shared it with, and why. 
  • We will include all the disclosures except for those about treatment, payment, and health  care operations, and certain other disclosures (such as any you asked us to make). We’ll  provide one accounting a year for free but will charge a reasonable, cost-based fee if you  ask for another one within 12 months.

  • Most sharing of psychotherapy notes 

 

Fundraising Communications: 

  • We may contact you for fundraising efforts, but you have the right to opt out of fundraising  communications at any time.
  • You may opt out by following the instructions included in any fundraising  communication, or by contacting us via the information listed at the top of page 1 of  this notice.

How do we typically use or share your health information? We typically use or share your health  information in the following ways:

 

Treat You 

  • We can use your health information and share it with other professionals who are treating  you.
  • Example: A doctor treating you for an injury asks another doctor about your overall  health condition.

Special Protections for Substance Use Disorder Records (42 CFR Part 2) 

  • If we provide substance use disorder (SUD) diagnosis, treatment, or referral for  treatment and are subject to 42 CFR Part 2, your SUD records receive additional  protections under federal law. 
  • In general, we may not use or disclose these records unless you provide written  consent or unless otherwise permitted by law. 
  • Under federal law, you may authorize a single written consent for the use and  disclosure of your SUD records for treatment, payment, and health care operations.  This consent remains valid until you revoke it in writing.
  • Certain disclosures — such as for law enforcement investigations, legal  proceedings, or the disclosure of counseling notes — require separate, specific consent unless otherwise permitted by law. 
  • If a breach of your SUD records occurs, we will notify you as required by law. 

 

Note: These additional protections apply only to records covered by 42 CFR Part 2. Not all  health information is subject to these rules.
